Data enabling this vision can include the collection of patient-reported outcomes and experiences through questionnaires and the data flow directly into the patient's clinical record and the physician and the patient review them together. Aggregated data about patient experiences and outcomes are shared among physicians for learning. Patient portal is available where patients review their own records and offer new information about their health and progress. The portal includes option to consult professionals when they need them - on demand e-consultation. 

To realise this vision in Switzerland we would need to transform the thinking within the existing medical hierarchy, within the health department and the social care department. Suggest opportunities for Swiss experts to learn from experiences in other countries, like Finland, where these digital tools and data are already used.
